ABSTRACT

Objective:To study the effects of enhanced breastfeeding management on the neurodevelopmental outcomes of preterm infants in neonatal intensive care unit (NICU).Methods:From January to December 2020, preterm infants with gestational age (GA) ≥32 weeks admitted to NICU of our hospital were randomly assigned into the enhance group and the control group. Breast-feeding education using WeChat and routinely playing recorded mother's sound to the infants were carried out in the enhance group, while conventional management was carried out in the control group. Breastfeeding status was followed up regularly in both groups. Mental development index (MDI) and psychomotor development index (PDI) at 6-month of corrected GA were measured using Infant Intelligence Development Scale.Results:A total of 109 cases were included in the enhance group and 118 cases in the control group. The breast-feeding rates in the enhance group were significantly higher than the control group at discharge, 3-month and 6-month of corrected GA [88.1% (96/109) vs. 61.0% (72/118), 90.8% (99/109) vs. 67.8% (80/118), 88.1% (96/109) vs. 61.9% (73/118)] ( P<0.05). MDI and PDI at 6-month of corrected GA in the enhance group were significantly higher than the control group [(106.2±10.7) vs. (101.9±11.4), (103.4±13.7) vs. (99.5±11.6)] ( P<0.05). Conclusions:Enhanced breast-feeding management is helpful improving the breast-feeding rate of preterm infants in NICU and beneficial to the neurodevelopment of preterm infants.

ABSTRACT

OBJECTIVE@#To explore the treatment methods and experience of open fracture of lower limb in high altitude area.@*METHODS@#From January 2016 to January 2021, 62 patients with open fractures of lower limbs were treated by staged surgery with the concept of injury control orthopedics, emphasizing wound treatment and combining various fracture fixation methods. There were 51 males and 11 females, ranging in age from 14 to 59 years old, with a mean of (37.2±12.3) years old; and the course of disease ranged from 7 to 59 days, with a mean of (23.7±15.5) days. According to Gustilo Anderson classification, there were 14 cases of typeⅠ, 24 cases of typeⅡ, 14 cases of typeⅢA, 8 cases of typeⅢB and 2 cases of typeⅢC. The fracture repair and wound healing were observed, and the clinical efficacy was evaluated by Johner-Wruhs evaluation standard.@*RESULTS@#Fifty-five patients were followed up, and the duration ranged from 4 to 36 months, with a mean of (14.7±8.5) months, and 7 cases were lost to follow-up. According to Johner-Wruhs evaluation criteria, 33 cases got an excellent result, 16 good, 4 poor and 2 bad. The wound healing was poor in 2 cases, partial necrosis of Achilles tendon in 1 case, nonunion of fracture in 1 case and delayed healing of fracture in 2 cases.@*CONCLUSION@#It is an effective method to treat the open fracture of lower extremity in high altitude area to pay attention to the management of soft tissue injury, the management of wound moisturizing, staged operation of fracture and full protection of blood supply at the fracture end. Paying attention to the treatment of soft tissue injury and the management of wound moisturizing, staged operation of fracture and full protection of blood supply at the fracture end are effective methods for the treatment of open fracture of lower limbs in high altitude areas.

ABSTRACT

One hundred and fifty diabetic patients,who received management by community family doctors and hospital consortium (1+1+1) team for at least 1 year,were enrolled from January 2017 to May 2017.After that they continued to receive the management for 1 more year by the team which were trained with TeamSTEPPS (team strategies and tools to enhance performance and patient safety).The management effect before and after TeamSTEPPS training was compared.After the TeamSTEPPS training,the team management ability was improved,so that the compliance of diabetic patients with medical advice,regular examination of blood glucose,regular follow-up assessment,health education,regular exercise and diet control were all better than before,and the differences were statistically significant (all P<0.01).Fasting blood glucose,postprandial 2 h-blood glucose,glycosylated hemoglobin and LDL-C levels were all lower than before,and the achieved rate of the above indicators were also increased (P<0.05).It is suggested that application of TeamSTEPPS in team training can effectively enhance the management ability for chronic disease of family doctors and improve compliance and clinical indicators of patients with diabetes under management.

ABSTRACT

Objective@#To explore the clinical and genetic characteristics of infantile nephrotic syndrome caused by COQ2 variants.@*Methods@#The clinical and genetic data of a patient with nephrotic syndrome caused by COQ2 variants diagnosed at pediatric department of Peking University First Hospital from February 2018 to March 2018 were retrospectively analyzed. Related literature retrieved from PubMed, CNKI and Wanfang databases were searched to date (up to July 2018) with "COQ2 gene" or "primary coenzyme Q10 deficiency" and "nephrotic syndrome" or "nephropathy" as key words.@*Results@#A 14-month-old male, presented to local hospital at 11 months of age with edema and severe proteinuria, without hematuria, hypertension or renal dysfunction. He did not have infection or seizure in the course of the disease. He had no response to a more than four-week full-dose prednisone treatment. He had normal birth, mild motor development retardation and moderate language retardation. He was born to non-consanguineous healthy parents. He had two unaffected older sisters and one older sister died of "nephropathy" at one year of age. Genetic testing identified compound heterozygous variants in COQ2 gene: c.518G>A and c.973A>G, both could be predicted by in silico tools to be deleterious in protein function. These variants are not single nucleotide polymorphism and rare in normal populations. Both variants have previously been reported as pathogenic. These missense mutations were inherited from parents in autosomal recessive manner tested by Sanger sequencing. The patient was supplemented with high-dose of coenzyme Q10, at 30 mg/(kg·day) and glucocorticoid was withdrawn. Within three weeks of high dose coenzyme Q10 treatment, the edema disappeared. After seven weeks of high dose coenzyme Q10 treatment, the patient had decreased proteinuria and improved serum albumin levels. The urine protein to creatinine ratio decreased from 22.87 mg/mg to 1.98 mg/mg; Serum albumin increased from 14.2 g/L to 39.9 g/L, with normal kidney function and improved motor development. Primary CoQ10 deficiency is reported to be a rare autosomal recessive mitochondrial disorder with heterogeneous renal, neurologic, and muscular manifestations. To date, COQ2 variants have been reported in 14 children with glomerular involvement. Their age at onset ranged from neonatal period to 10-year-old (8 patients within the first year of life). Steroid resistant nephrotic syndrome (SRNS) is the most common phenotype. Some of these children also had progressing encephalopathy and myopathy, and seizures. Patients with COQ2 variants might show clinical improvement with early high-dose oral CoQ10 supplementation. Literature review revealed two Chinese articles, mainly about adults with neurologic symptoms. SRNS was previously not reported in Chinese pediatric patients.@*Conclusions@#It is necessary to carry out genetic testing for infant with SRNS. The coexistence of some degree of encephalomyopathy, such as development retardation, should raise suspicion of a mitochondrial defect caused by COQ2 variants. Timely diagnostic genetic testing and early high dose of coenzyme Q10 supplement could significantly improve their prognosis.

ABSTRACT

Objective To compare the clinical effect of two-person operations of disposable circumcision stitching instrument and one-man operations of disposable circumcision stitching instrument.Methods Clinical data of 340 patients with redundant prepuce or phimosis from March 2015 to August 2017 were randomly divided into control group which patients with two-person operation of disposable circumcision stitching instrument and study group which patients with one-man operation of disposable circumcision stitching instrument were reviewed prospectively.There were 188 patients in the study group,including 22 patients with phimosis and 166 patients with redundant prepuce;Among the 152 patients in the control group,there were 17 cases with phimosis and 135 cases with redundant prepuce.Two groups with operation time,intraoperative blood loss,postoperative skin staple missing,2 hours postoperative pain,postoperative edema,the swelling subsided time,postoperative complications,postoperative patient satisfaction,and so on.Measurement data were represented as (x) ± s,and comparison between groups was analyzed using t test;count data were represented as percentage and comparison between groups was analyzed chi-square.Results All operations were finished successfully for the patients by two-person or one-man of operations of disposable circumcision stitching instrument.In terms of operation,the operation time of the observation group and the control group was (7.76 ±0.45) minutes and (7.86 ±0.91) minutes respectively,and the difference was not statistically significant(P > 0.05).Intraoperative hemorrhage of observation group and control group were (1.77 ± 0.22) ml and (1.72 ± 0.26) ml,and the difference was not statistically significant (P > 0.05).There was no statistically significant difference between the observation group and the control group (P > 0.05) in the postoperative 2 hours pain score and skin staple missing.In the postoperative complications,the deviation rate of the prepuce was 1.11%,significantly lower than the control group 5.92%,and the difference was statistically significant (P < 0.05).There was no statistically significant difference between the observation group and the control group in terms of edema on the third postoperative day,short postoperative anastomosis,patient satisfaction,and postoperative infection.Conclusion The way of one-man operation of disposable circumcision stitching instrumente not only saves labor costs,but also has a good postoperative clinical effect,meanwhile,it has high efficiency and strong operability,and is worthy of promotion for clinical first-line urologists.

ABSTRACT

Objectives:To analyze the clinical value and safety of total laparoscopic gastro-duodenal triangle anastomosis in radical surgery of distal gastric cancer.Methods:Sixty patients with gastric cancer treated from May 2013 to August 2016 were selected as study subjects and randomly divided into experimental group and control group,with 30 cases in each group.The experimental group was given laparoscopic radical gastrectomy for gastro-duodenal anastomosis,while the control group was given laparoscopic auxiliary distal gastrectomy for radical gastrectomy.The clinical efficacy and safety of the two groups were compared.Results:Compared with the control group,the operation time of the experimental group was longer,the blood loss was less,the distal margin of the tumor was longer,the postoperative pain score was lower and the dosage of analgesics was less (P<0.05);There was no significant difference in TNM stage and Lauren classification between the two groups(P>0.05).The incidence of postoperative complications in the experimental group was 6.67%,which was lower than that in the control group(10.0%),but the difference was no Statistical significance (P>0.05).Conclusion:Total laparoscopic distal gastric cancer Gastro-duodenal triangle anastomosis is effective and safe,and worthy of clinical application.

ABSTRACT

Objective To explore the effects of extension nursing on quality of life in cervical cancer patients after radiotherapy. Methods A total of 80 hospitalization patients from January 2013 to January 2015 was selected to join the QQL-BREF test. These 80 patients were divided into the experimental group and the control group, who all accepted regular health education, and the experimental group also accepted nursing once a month after discharge. The quality of life of patients in the two groups were assessed one year later. Results For the domain of physiological field, psychological field, independence, social, environmental, faith, the experimental group scored 3.73 ± 0.33, 3.66 ± 0.44, 3.67 ± 0.05, 3.48 ± 0.59, 3.73 ± 0.58, while the control group scored 1.68 ± 0.40, 2.39 ± 0.43, 1.76 ± 0.39, 2.10 ± 0.51, 3.02 ± 0.45 (t=1.26-21.15, P 0.05), Other 17 elements showed statistical difference (t=2.45-17.31, P < 0.01). Conclusions Extension nursing can improve cervical cancer patients′quality of life efficiently after radiotherapy and reduce the recurrence rate.

ABSTRACT

Objective To study the influencing factors of digital image white balance of the biological microscope and the way to obtain optimized image.Methods Color temperature meter was used to measure the influences of the factors and their correlations, and SASS software was employed for statistical analysis.Results Object lens might increase the color temperature by 200 to 300 K, LBD color filter could enhance it by 400 to 1 800 K. Reducing field stop by 90% might decrease color temperature by 400 K. ND6 filter, ND25 filter and aperture diaphragm had no influences on color temperature.Conclusion The electric and optical parameters of biological microscope have to be adjusted to gain proper white balance for digital image.

ABSTRACT

Object To investigate the correlation of IGF-2 exon 9 CpG island methylation and the expression of IGF-2,and further explore the mechanism of Wilms tumor. Methods The IGF-2 exon 9 methylation status and IGF2 gene expression in 42 cases of Wilms tumor and corresponding normal tissues were detected by using methylation-sensitive restriction enzyme PCR and allele-specific IGF2 gene expression analysis.Results IGF-2 exon 9 methylation rate was 15% in W ilms tumor group, and 97% in normal tissue group. The difference was significant (P < 0.01), and the difference in pathological type was also significantly different (P < 0.01). Exon 9 unmethylated tissues showing biallelic expressions of IGF-2 were significantly higher than that of methylated tissues (P < 0.05). Conclusion IGF-2 exon 9 unmethylation may be the cause of biallelic expression of IGF, which contritues to tumorigenesis in Wilms tumor.

DNA methylation is a way of regulating gene expression of eukaryotes. DNA methylation in the regulation of normal cell development and cancer play an important role. There is evidence that in cancer patients compared with normal subjects,the former showed higher tumor suppressor gene methylation and cancer gene abnormality hypomethylation. In a variety of cancer types, related genes that can control cell cycle, proliferation, apoptosis ,metastasis,drug resistance and intracellular signal transduction exsist abnomal methylation.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the effects of continuous blood purification on the hemodynamics and oxygenation in patients with acute respiratory distress syndrome (ARDS).</p><p><b>METHODS</b>Twenty-one patients with ARDS were treated with continuous veno-venous hemofiltration (CVVH) combined with plasma exchange. Hemodynamics and oxygenation were measured or calculated at scheduled intervals using Swan-Ganz catheters.</p><p><b>RESULTS</b>The mean arterial pressure, partial pressure of arterial oxygen, oxygen delivery, oxygen consumption increased, heart rate, mean pulmonary arterial pressure, pulmonary capillary wedge pressure, blood lactate concentration all decreased significantly after the treatment, and the oxygen extraction ratio underwent no obvious changes.</p><p><b>CONCLUSIONS</b>Continuous blood purification can increase blood and oxygen supply but has no significant effects on oxygen extraction ratio in ARDS patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of microfilament cytoskeleton changes induced by simulated microgravity on the NO/NOS system of osteoblasts, and explore the mechanism of weightlessness leading to osteoporosis.</p><p><b>METHODS</b>Mouse osteoblast-like cell line MC3T3-E1 were divided into simulated microgravity (by rotating clinostat) group, 0.5 microg/ml cytochalasins B group, simulated microgravity+cytochalasins B group, and normal gravity group. After cell culture for 48 h with corresponding treatments, immunofluorescence staining of the cells by FITC-phallacidin was performed to observe the changes of microfilament under laser confocal microscope. NOS activity of the cells was tested with NOS detection kit, and the NO concentration in the cell supernatant was measured with nitrate reductase method.</p><p><b>RESULTS</b>Depolymerization of the microfilament cytoskeleton with irregular arrangement and reduced tension fibers occurred in the cells except for those in the normal gravity group, which was especially obvious in the microgravity+cytochalasins B group. Compared with the normal gravity group, the cells in the other groups showed increased iNOS activity and NO concentration but decreased eNOS activity, especially obvious in simulated microgravity+ cytochalasins B group.</p><p><b>CONCLUSION</b>Disruption of the microfilament cytoskeleton induced by simulated microgravity can regulate the NO/NOS system to influence the signal transduction of the osteoblasts.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare macroporous microcarrier Cytopore 2 and Cultispher S for their effects in microgravity culture of CL-1 cells.</p><p><b>METHODS</b>CL-1 cells were cultured on Cytopore 2 and Cultispher S respectively in a rotational cell culture system (RCCS) in a volume of 50 ml. Dynamic morphological observation and cell counting and functional test were carried out during the cell culture.</p><p><b>RESULTS</b>The cells were capable of adhering to and proliferating on both of the microcarriers, reaching the growth peak on day 9 of cell culture with the maximum cell density of 4x10(6)/ml on cultispher S. Albumin was significantly higher in the supernatant of Cytopore 2 than in that of Cultispher S on days 10, 11, and 12 (P<0.05), and the urea level in the supernatant of cytopore 2 was also significantly higher on days 10 and 11 (P<0.05).</p><p><b>CONCLUSION</b>The cells cultured on Cytopore 2, though with a smaller cell number, display better functions than those cultured on Cultispher S under RCCS conditions.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the effect of artificial and bioartificial liver support systems for management of acute and acute-on-chronic liver failure.</p><p><b>METHODS</b>Articles documenting randomized clinical trials concerning any liver support systems vs standard conservative therapy, published between January, 1970 and June, 2008, were retrieved by database searching. Of the 1134 articles retrieved, 12 randomized trials involving 479 patients were included. The data were extracted and the trial quality was assessed by 2 independent reviewers. The primary outcome measure was all-cause mortality, and the results were combined on the risk ratio (RR) scale.</p><p><b>RESULTS</b>Of the 12 trials included, 10 assessed artificial liver support systems for acute or acute-on-chronic liver failure, and 2 assessed bioartificial systems for acute liver failure. Overall, the liver support systems had moderate effect on mortality compared with standard conservative therapy (RR=0.80; 95% CI 0.664-0.969, P=0.022). Meta-regression indicated that the effect of the support systems depended on the type of liver failure (P=0.00). In stratified meta-analyses, the support systems appeared to reduce the mortality by 43% in acute-on-chronic liver failure (RR=0.57; 95% CI 0.39-0.84, P=0.004), but not in acute liver failure (RR=0.899; 95% CI 0.72-1.12, P=0.361).</p><p><b>CONCLUSION</b>Artificial liver support systems reduce the mortality of acute-on-chronic liver failure as compared with standard conservative therapy, but have no significant effect on the mortality of acute liver failure. Bioartificial liver support systems lower the mortality rates in both acute and acute-on-chronic liver failure, and should be the future focus of development.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To assess the sensory integration function of children with primary nocturnal enuresis (PNE) and explore the role of sensory integration dysfunction in the pathogenesis of PNE.</p><p><b>METHODS</b>Sensory integration function was assessed by the Childhood Sensory Integration Ability Development Checklist in 46 children with PNE and 46 normal children (control).</p><p><b>RESULTS</b>The incidence of sensory integration dysfunction in the PNE group (82.6%) was significantly higher than that in the control group (43.5%)(P<0.01). Seventeen patients (36.9%) presented with severe sensory integration dysfunction in the PNE group but only 1 (2.1%) in the control group (P<0.01). The scores of all nine sensory integration indexes revealed by sensory integration function testing in the PNE group were significantly lower than those in the control group (P<0.01).</p><p><b>CONCLUSIONS</b>Children with PNE have sensory integration dysfunction. Sensory integration dysfunction may be associated with the pathogenesis of PNE.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To screen the mutations of the low density lipoprotein receptor (LDLR) gene in a familial hypercholesterolemia (FH) family, and analyze the LDL-uptaking function of LDLR on lymphocytes of patients.</p><p><b>METHODS</b>Genomic DNA was extracted from four affected members in a Chinese FH family. The presence of apoB100 gene R3500Q mutation which results in familial defective apolipoprotein B100 (FDB) was excluded first. Fragments of the LDLR gene were amplified by touch-down polymerase chain reaction (Touch-down PCR) and analyzed by single-strand conformational polymorphism (SSCP). The suspect fragments of the LDLR gene were cloned and sequenced. Furthermore, the lymphocytes bounded with fluorescent-labeled LDL (DiI-LDL) were measured by fluorescence flow cytometry.</p><p><b>RESULTS</b>A nonsense mutation was identified in exon 10 of LDLR gene. This mutation gave rise to a premature stop codon (W462X), resulting in the absence of most of the LDLR domains. It was detected in all the affected members of the FH family. The ratios of functional LDLR in lymphocytes from patients and normal controls were 63.7% and 77.3% respectively. As a result, the activity of the functional LDLR in patients was just 82.4% of that in the normal controls.</p><p><b>CONCLUSION</b>It is possible that the W462X mutation of LDLR gene is the main cause for the disease in this family.</p>

ABSTRACT

Classical swine fever (CSF) is a contagious swine disease charactered by hemorrhagic fever and leukopenia,usually leading to substantial economic losses. To obtain a insight of leucopenia caused by CSFV infection, DNA microarray analyses of peripheral blood leucocytes (PBL) of the infected pigs was performed. Three health pigs were inoculated with a lethal dose of CSFV Shimen strain and their PBLs were isolated when the onset of typical clinical signs and then subjected to total RNA extraction followed by microarray analysis with Affymetrix Porcine Genome Array GeneChips. The results showed that the significant differences were observed in cellular apoptotic genes expression at 7 days post-infection (p. i.). The changes of the genes expression were confirmed by real time RT-PCR of some selected apoptosis-related genes. This study provided a valuable information for further investigating the molecular mechanism of apoptosis caused by CSFV infection.

ABSTRACT

<p><b>OBJECTIVE</b>To study the endogenous hormone of testosterone and cortisol that secretes volume and rhythm in sports fatigued human bodies and animals. To determine secretory volume and rhythm in sports fatigued human bodies and animals when Shixiang yaofa's drug are used.</p><p><b>METHOD</b>Radio-immunity method was used to determine secretory volume and rhythm in sports fatigued human bodies and animals and to analyze secretory volume and rhythm. According to the secretory volume and rhythm of testosterone and cortisol, the Shixiang Yaofa's drugs were used. Doses: wenyang jihuo bead 10 g/sack 2 sack, ziyin xiuyang capsule 0.5 g/pill 8 pill pd in human bodies for 28 days. Wenyang jihuo bead 10 g x kg(-1) of crude drug, ziyin xiuyang capsule 4 g x kg(-1) of crude drug pd for hight doses in animals. Other groups for low doses 5 g x kg(-1) and 2 g x kg(-1) of crude drug pd for 15 days. The blood samples were collected for determination.</p><p><b>RESULT</b>(1) In human bodies the peak value of testosterone was appeared in 8:00 and valley value was appeared in 18:00, ranges: 176.93-281.73 x 10(-5) mg x L(-1). The peak value of cortisol was appeared in 8:00 and valley value was appeared in 22:00, ranges: 1.31-16.13 x 10(-3) mg x L(-1). In the same condition, the mouse peak value of testosterone was appeared in 20:00 and valley value was appeared in 0:00, ranges: (0.56 x 10(-5) - 124.0 x 10(-5)) mg x L(-1). The rhythm in animals was compatible with human bodies, howerer, the peak value was delayed for 12 hours. (2) The testosterone was step up and the cortisol was cut down in sports fatigued human bodies and animals when shixiang yaofa's drug were used (P < 0.05, P < 0.01).</p><p><b>CONCLUSION</b>The secretion of testosterone and cortisol in sports fatigued human bodies and animals are existed conclusive biologic rhythm. The secretory volume can be available accommodation by shixiang yaofa's drugs.</p>

ABSTRACT

Cryptorchidism is a clinically common congenital deformity of the genital organ, the etiological factors of which are still not clarified up to now. Undescended testis is frequently accompanied with the anomalies of the spermatic cord, gubernaculum testis, processus vaginalis, and epididymis. Because these anatomical anomalies act as terminal factors or the ending in the links of the cause of cryptorchidism, it is of important directive significance for the clinical treatment of the problem to seek evidence of anatomical anomalies at orchiopexy to demonstrate the etiological factors of cryptorchidism.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the bracket bond failure and its causes between adult and adolescent patients during fixed orthodontic therapy.</p><p><b>METHODS</b>Bracket bond failure data of 30 adults and 30 adolescents, receiving fixed orthodontic therapy, have been collected within the first 12 visits, respectively. The compliance has been analyzed with survival analyse between the two groups.</p><p><b>RESULT</b>The general bracket bond failure rate in the adult group is lower than that of the adolescent group and the difference is significant (p < 0.05). In the adolescent group, the failure rate for mandibular anterior teeth is highest and different from that of the adult group (p < 0.05). The failure rate resulted from biting hard food is ranked No. 1.</p><p><b>CONCLUSIONS</b>The compliance of the adults receiving fixed orthodontic therapy is better than that of the adolescents. The investigation of bracket bond failure causes is beneficial in helping orthodontists improve orthodontic practice and raise clinical efficiency. The survival analysis is effective in evaluating the bond failure.</p>
